At ET Mutt Hutt, every dog that comes through our door is treated like family — because to us, they are. Before your pup's first stay, we just need a few minutes of your time to get to know you both a little better.

Filling out the form below helps us take the best possible care of your dog from the moment they arrive: their favorite food, their quirks, anything that makes them them. It also covers our boarding agreement, so we're all on the same page before the fun begins.

Here's what to have handy:

• Your contact info
• A little about your dog — breed, age, food, any medications or things we should know
• A photo of your pup (we love putting a face to the name!)
• A copy of their current vaccination record
